This intake provides enough context to determine whether coaching is a good fit and what the next step should be. Completing it does not commit you to a monthly plan.

Health & Training History
Privacy note: Health information you share is used only to determine whether coaching is appropriate. Do not include highly sensitive information unless it is genuinely necessary. All submitted information is handled securely.
Readiness Screening
The following questions help determine whether medical clearance may be appropriate before starting coaching. This is not a substitute for the official PAR-Q+ screening tool.
Important: If you answer yes to any of the following questions, Dakota may request medical clearance from your physician before beginning coaching.
If you answered yes to any question, Dakota recommends consulting your physician before starting coaching. The official PAR-Q+ screening tool provides a more complete assessment and can be found at eparmedx.com.
Assessment Preparation
Here is what to expect if you and Dakota decide to schedule a Two-Hour Discovery & Assessment.
Location
Dakota is based in Black Mountain and serves the greater Asheville area. Exact locations depend on the type of coaching and current facility arrangements.
What to wear
Comfortable training clothes you can move freely in. Athletic shoes or barefoot depending on the setting.
What to bring
Any relevant medical records, previous training logs, or sport-specific footage. Water and a snack if needed.
How long to reserve
Up to two hours. The session may conclude earlier if the necessary information has been collected.
Hydration & meals
Eat normally and hydrate well before the appointment. Avoid training hard immediately before the assessment.
Sport footage
If you climb, run, or play a sport, bring video footage if available. It helps Dakota understand your movement in context.
Cancellation policy
Sessions canceled with at least 24 hours' notice may be rescheduled. Late cancellations and no-shows generally count as completed sessions.
After the appointment
Dakota prepares a brief written Coaching Map with initial priorities and a recommended service tier. Coaching can begin from there.
